Title : Revolutionizing heart health through Personalized & Precision Medicine (PPM): Towards personalized cardiovascular prevention, precision cardiac care & rehabilitation via optimizing Cardiac Hospital Networks
Abstract:
Understanding personalized & precision cardiology (PPC) focuses on customizing medical interventions based on an individual’s unique genetic profile, biomarkers, targets and personal health data. Unlike conventional cardiology, which relies on generalized treatment guidelines, PPC-guided approaches take into account genetic variations that may influence a patient's response to medications and disease progression. The field integrates genomic testing, digital health technologies, and IT-driven predictive analytics to create individualized treatment plans that enhance the effectiveness of cardiovascular care and post-treatment rehabilitation.
Testing genetic predisposition (comprehensive genome profiling) plays a crucial role in the development of cardiovascular diseases (CVDs). Advances in genome sequencing, genomic microassay assays, GWAS screening and polygenic risk scoring enable pre-early (subclinical) detection of specific and profiled biomarkers and inherited conditions such as hypertrophic cardiomyopathy, familial hypercholesterolemia, and arrhythmias. Genetic testing allows physicians to identify individuals at high risk and implement preventive strategies tailored to their genetic profiles.
It would be extremely useful to integrate data harvesting from different databanks for further treatment to thus provide more tailored measures for the patients and pre-illness persons-at-risk, resulting in improved patient outcomes, reduced adverse events, and more cost effective use of the latest health care resources including diagnostic (companion theranostics), preventive and therapeutic (targeted molecular and cellular) etc. In this context, the need for innovative heart disease treatments has become critical since the diseases remain the world’s biggest killer. And the pace of innovation in PPC is becoming fast.
A major paradigm shift has become the increasing recognition of the potential therapeutic utility of the targeted drugs for cardiovascular diseases, whilst opening up new avenues of therapeutic implications. In cardiovascular disease globally, PPC may result in much-needed innovation in the field and has the potential to eventually change the way we treat heart diseases altogether. So, PPC is thus poised to be-come the next great revolution in the daily practice, as well as in the maintenance of cardiovascular health and the prevention and cure of cardiovascular disease.
The promise of PPC is well understood but the newest tools will be needed for describing the cardiovascular health status of individuals and populations, including transdisciplinary 'OMICS' data, exposome-related datasets and social determinants of health, behaviours and motivations, patient-generated data, and the array of data in EMRs. Innovative cardiology and cardiac nanosurgery currently lead the way in PPC advancements, and health care under PPM-related armamentarium will become a more integrated, dynamic system, in which patients are becoming central stakeholders who contribute data and participate actively in shared decision-making. The proposal would ideally be suited for practitioners who already incorporate integrative approaches in their practice, as well as more traditional cli-nicians who want to learn more about PPM and PPC as a growing area.
Along with PPC-guided treatment protocols, cardiac rehabilitation prepares patients for safe recovery after a heart condition or procedure through a personalized plan that combines medical evaluation, supervised exercise, education, and lifestyle support, whilst assembling the multidisciplinary cardiac care & rehabilitation team. Precision cardiac rehabilitation represents a pivotal approach in the secondary prevention of CBDs, yet conventional assessment and intervention models are limited by their static and homogeneous nature. By bridging IT supportive multi-OMICS insights with clinical practice, this dynamic navigation framework promises to transform precision cardiac rehabilitation and improve patient outcomes.
PPC will need to demonstrate that phenotype-based person-specific interventions are superior to the current standard of care and, ultimately, have a population effect by moving the mean on the disease spectrum towards Health. This is the reason for developing global scientific, clinical, social, and educational projects in the area of upgraded clinical cardiology to elicit the content of the new branch. In this context, cardiac networks aim to align and integrate efforts in CVDs to accelerate translation of discovery to improvements in care.
