Dear congress visitor it is an honor and a pleasure to write a few welcome notes. Disturbance of microcirculation in the musculoskeletal system is a prime cause of pain and injury. It has been shown that chronic muscle spasm impedes skeletal muscle microcirculation and results in loss of mitochondria and muscle fibers. It has been further shown that resolution of the chronic muscle spasm allows the muscle to regenerate mitochondria and muscle fibers. Disturbances of microcirculation appear to also play a role in the development of tendinopathy. The microcirculation of tendons is easily impaired by chronic tension cause by chronic spasm of the attached muscles. I will share with you the findings of my clinical research over the past 15 years and the theories that developed to explain those findings.

Dear Congress Visitors,

I am delighted to welcome you to the International Heart Congress. Clearly, the heart is one of the most important organs in our body. However, it is also one of the most problematic organs in the body. According to the World Health Organization, cardiovascular diseases (CVDs) are the leading cause of death globally with an estimated 17.9 million people who died from CVDs in 2019. Of course, with COVID following shortly thereafter, such statistics (including a drop in global life expectancy) have only become worse. Alarmingly, CVDs represent 32% of all global deaths. If our efforts over the last century have helped prevent, diagnose, and treat CVDs (which I think they have), why are so many people still dying from CVDs ? Are our approaches focused correctly ? At the right people ? At the right communities ? Let me give you my thoughts.

While it is true that many traditional societal conferences have been devoted to CVDs, this congress is different. This International Heart Congress has contributions from all over the world, fit for all over the world. I personally am always disappointed to see the same presentations from the same people at well-established societies or conferences – it is truly the “all boys network”, usually from wealthy countries receiving grants from funding agencies that prohibit reaching a truly global population. While such research is important, such talks are usually not focused on CVD solutions that can benefit the entire world. Did you know that, as true with other diseases, those from under-developed countries suffer CVDs the most accounting for over 75% of the total CVD deaths globally ? We have to transition our thinking to helping everyone, not just the country we are from – only then will we see global decreases in CVD. We need to ask ourselves the following questions: Does our research address CVD healthcare to those with limited sources ? Geographical restrictions ? No healthcare at all ? How can we be solving CVDs without reaching the patients that need them most ? What can you do ? This is why I personally have committed to this fight and established a new research fund for Nigeria and other African countries to explore the best healthcare options for CVDs.

These and other new ground-breaking issues in global CVDs will be discussed at this International Heart Congress. Join me in developing new strategies to prevent, diagnose, and treat CVDs for everyone. Set your goal that can help once and for all make CVDs a statistic from the past, not the present, and certainly not the future. Set your goal to reduce CVDs where 75% of all CVDs occur, only then, can we see a change in CVD statistics.

Cardiovascular disease continues to lead the way in complications and deaths among adults around the world. In developed countries, the incidence rate has decreased, while in developing countries, these rates are somewhat different. As data accumulates, international recommendations are updated. It is safe to say that international treatment standards are applied in all countries. One of the themes of this conference is that of myocardial ischaemia and infarction. At first glance, this would seem to represent purely a repetition of our previous focus on the traditional theme of a common spectrum of cardiovascular disorders driven by the traditional risk factors of hypertension, smoking, diabetes and poor diet, and emerging mainly in middle-aged men. Our therapeutic discussion 30 years ago would have emphasised coronary revascularization above all. However, much has changed. In Western societies, emergence of coronary atheroma-related disease has generally been delayed, if not prevented, while an emerging epidemic threatens the Third World. We have finally learned the importance of inflammation as a key component of the pathogenesis of atherogenesis and myocardial Welcome Message ischaemia, and are beginning to refine our pharmacotherapy to accommodate the recognition of its potential effectiveness and the emergence of “new” forms of acute heart disease which occur mainly in women. The field of cardiovascular biomarker research is entering a new, exciting era with the introduction of the concept of ‘personalized medicine’ in therapeutic treatment guidance. With a variety of new cardiovascular biomarkers, and assertions of their respective independent associations with outcome, it becomes increasingly difficult to select the biomarker with the strongest diagnostic as well as prognostic value. One can expect that the increasing use of cardiovascular biomarkers, accompanying traditional diagnostic and prognostic measures, for specific cardiovascular diseases, will lead to their almost crucial role in clinical decision making. However, the search for an ideal biomarker, or set of biomarkers, requires a systematic and rigorous approach to biomarker studies, more emphasis on Welcome Message prospective studies, and use of external validation cohorts. Accomplishment of the previous will lead to improved quality of diagnostic and, even more important, prognostic biomarker research in the future, and will be required if we want to bring this clinically important research field forward.
